If these payments were described by a different name other than "buy-out" please indicate the label used and how much the Town of Wappinger paid Steven Frazier annually not to utilize the town's medical/health insurance plan for the aforementioned years. Payment amount listed byyear is requested. In an effort to reduce the amount of paper utilized, emailed copies of official notices of payment with confirmation of receipt are requested. If unavailable, an official notarized verification from the Town of Wappinger Town Clerk indicating the amount paid per year to Mr. Frazier will suffice.
