Weston & Sampson is preparing a Phase l Environmental Site Assessment (ESA) far a parcel located
at 15 Middlebush Road (Parcel ID: 135689-6157-01-324814-0004), on behalf of the Wappingers
Central School District.
As a part of the assessment, we are required to request the following information from the Town of
Wappinger regarding the property, if possible:
Office of the Assessor
Assessor's Map/ Card
• info on building sizes, tot sizes, tax map & lot number
• Current and former owner info
• Title/deed records
Office of Code Enforcement
• Permit records of any USTs/ASTs (underground/above-ground storage tanks)
• Tank installations and/or removals (dates, size of tanks, contents of tanks, locations)
• Building permits for demolition and construction
• Construction and demolition information
• Dates of original building construction
Fire Department
• Records of any USTs/ASTs (underground/above-ground storage tanks)
• Tank installations and/or removals (dates, size of tanks, contents of tanks, locations)
• Records of anyfires/incidents
