Dear Supervisor Smith & Board Members:
This report is written to update you on the events that have taken place
during the above referenced project, and to provide an update as to the
current conditions of the projects as per Monday, June 22nd, 1998.
MAY 18TH, 1998:
On May 18th, 1998, this office met with Gerhardt Rodenburger, Project
Manager for New Hope Pipe Liners, Inc., and Dare Fitizpatrick from Camo
Pollution Control, at the site for a pre-construction meeting regarding the
project. At this meeting, we were informed by the New Hope Pipe liners
representatives the entire lining process from start to finish would take
approximately 10-12 hours to complete. The liner would be installed
within 1-2 hours with the curing process taking 4-6 hours. The laterals
would then be reinstated to allow service to continue within a time frame
of 2 - 3 hours.
We asked Mr. Rodenberger if there would be any problems with plugging
the Kent Road manhole even though a force main discharge flows into this.
He informed us that that would not be a problem and that they would use
a flow through plug and bypass pumping would be provided to allow them
to continue their work.
He also indicated that the work would commence at 7:00 a.m. and
proceed until 6:00 p.m. thereby utilizing the time when most residents
were at work, and therefore, would not be using a large amount of water.

We then asked if the re-lining work could be performed from one manhole
(specifically the one at the corner of Kent Road and All Angels). He
indicated that that would be their procedure, and there would be no need
to enter the intermediate manhole along this pipe run which was in the
front yard of 228 All Angels Hill Road.
We also went over the required traffic control, including flag people to
allow the work to proceed if cars were parked along All Angels Hill Road.
They did not have a problem with that. Camo Pollution Control seemed to
be satisfied leaving the flow through plug in and not causing any problems
with their force main discharge.
All parties at the meeting seemed satisfied. The meeting was completed
with the Contractor indicating he would need approximately 2-3 weeks to
order and receive the liner prior to commencement of work. I asked him to
keep me informed of the status, and asked him to see if he could get this
material as soon as possible so we can being this project.
On June 12th, 1998, the Contractor sent out a TV and cleaning crew to
start cleaning the sewer main, and also do the camera work to locate and
document the exact location of the sewer laterals. Our inspector Jim
Majors was on the site at the time, and also viewed the TV camera work
that was performed in this pipeline. This work was completed at
approximately 2:00 p.m. Friday afternoon and the Contractor stated that
they would be back on Monday to finish the cleaning and TV work prior to
starting to install the liner.
JUNE 15TH, 1998:
On Monday, June 15th, 1998, the Contractor came with full crew including
TV and cleaning equipment, and boiler truck which is used to cure the
liner. The started work at approximately 9:00 a.m. with the cleaning and
camera work.
During the cleaning portion of the work, the Contractor needed water for
his cleaning truck. Camo Pollution Control allowed them to fill their water
truck from the Hilltop Water Plant site. With the water truck filled, the
Contractor then proceeded to pull off of All Angels Hill Road to the
intermediate manhole, which was in the middle of a homeowner's front
yard. The truck proceeded to become stuck in the front yard and could not
be backed out.

The homeowner then came out, and asked why this truck was parked
there. She had called the Town and the New York State Troopers due to
this incident. We spoke to the resident and explained that we would get
the truck out of the yard as soon as possible.
A New York State Trooper came on site. I explained the situation and the
happenings that had occurred up to that point, and that the Contractor
was now calling for a wrecker to come and pull the truck out of the yard.
Once the truck was removed, we inspected the damage. There appeared
to be two wheel ruts in the yard. The Contractor filled in the holes with
topsoil, raked the area and seeded.
However, by the time the truck was removed and cleaning operations
commenced again, they could not start the liner due to the fact it was too
late in the day, and they would not be able to get the liner installed and
cured in time for the residents to have sewer service the following
morning.
The Contractor then informed us that the lining work would commence
tomorrow, Tuesday, June 16th, 1998.
JUNE 16TH. 1998:
On June 16th, 1998, the Contractor was ready to work at 8:00 a.m. He
performed some preliminary TV and cleaning work and then set up for the
liner installation.
At 11 :00 a.m., the tower truck setup over the manhole at Kent Road and
worked proceeded. The tower trucked was raised to get the platform up
high enough to insert the liner. However, our Inspector then stopped work
shortly after 11 :00 a.m. because of the tower truck's proximity to the
Central Hudson power lines. Jim Tremper, Camo Pollution, called Central
Hudson and asked them to come to site, which in turn the utility company
came and shielded the lines. Once the lines were shielded the work then
continued.
The insertion of the liner started at approximately 1 :00 p.m. and was
completed at 2:30 p.m. However, during the installation process a tear at
the end of liner was noticed, and the Contractor needed to make a repair.

In order to make the repair, the Contractor had to reduce the amount of
water pressure on the liner at that particular time in order to get the repair
clamp over the area of the tear. The tear was repaired and the curing
process was started at approximately 3:30 p.m. on Tuesday, June 16th,
1998.
At 6:00 p.m. that evening, Mr. Ames, resident at the corner of Kent Road
and All Angels Hill Road, complained about wheel ruts that were in the
yard due to the tower truck pulling onto the edge of his yard off the road.
This area appears to be in the Town right-of-way, however, we informed
the Contractor that these ruts would have be repaired.
Also at 6:00 p.m. a complaint about water in the basement was issued by
the homeowner at 226 All Angel Hill Road. The Contractor then informed
the resident that the service would be reinstalled by 8:00 p.m. At 9:00
p.m., the resident filed another complaint. Our Inspector, Jim Majors, told
a laborer to inform the foreman. The foreman was not present on site.
The laborer told the homeowner that service would be back in one hour.
At 11: 15 p.m., the homeowner at 226 All Angels Hill Road had complained
again that no one cleaned up the backup of water in the basement. Our
inspector located the foreman and instructed him to see the resident. At
1 :45 a.m., the Contractor's foreman finally went to said homeowner and
mopped up the backup of water in the basement and promised again to
have service reinstalled by 6:00 a.m.
JUNE 17. 1998:
At 2:00 a.m., the homeowner of 224 All Angels Hill Road asked if service
would be back on line by the morning. The foreman informed the owner
that service would be back by 6:00 a.m.
Mr. AI Belarge, at 232 All Angels Hill Road, informed us Wednesday, June
17th, 1998, when he returned at 5:00 p.m. the night before he noticed that
water was backing up in his house, and at 7:00 p.m. - 8:00 p.m. flooding
begin in the 1 st floor bathtub. He indicated that he notified one of workers
for the Contractor and told him he was having backup of water into his
bathroom. However, the worker did not notify our inspector or their
foreman of this problem. Mr. Belarge then said he called the emergency
phone numbers for New Hope Pipe Liners that were on the notification
letter that was sent to all the Homeowners in the area about the project.

He said that after trying all the numbers, he received no response from
New Hope Pipe Liners.
JUNE 17TH, 1998:
Supervisor Smith, Michael Tremper of Camo Pollution Control, and this
office met with Mr. Belarge on Wednesday, June 17th, 1998 to look at the
damage and to talk to him about what had transpired through the evening.
This office then went to other residents along All Angels Hill Road involved
in the project area to see if they were experiencing any problems in their
homes. We did find a small backup in the bathroom of 224 All Angel Hill
Road, however, it was a small amount of water which removed one floor
tile. The homeowner cleaned the area and indicated that they would
replace said tile.
The homeowner of 226 All Angels Hill Road had showed me the area of
the leak into his basement, which the workers had cleaned at 1 :00 a.m..
The conditions included wet sheetrock, continuous water on the floor, and
it appeared that backup occurred in the slop sink which was in the
basement.
The homeowner of 228 All Angels Hill Road could not be reached for a few
days, due to no one was home at the time. However, when they were
contacted there was no backup problem in this house.
We then visited the homeowner at 230 All Angles Hill Road and were told
that there was flooding in the downstairs bathroom from the toilet and into
the tub. This backup then leaked from the bathroom through the common
wall into the garage, which sustained some damage to the sheetrock and
tile around the toilet area.
We then spoke to the homeowner at 234 All Angels Hill Road on
Wednesday, at 1 :35 p.m., and indicated that he had no problems up to
that time.
The homeowner at 207 All Angels Hill Road experienced a slight backup in
the downstairs bathroom which ruined the rug in the bath. The
homeowner removed the rug and there was no apparent damage to the
floorboards underneath.

We then continued to monitor the individual homes on both sides of All
Angels Hill Road to update any further problems or backups.
The Contractor was now trying to reinstate the service connection along
the pipe route. He had difficulties getting through some lateral at which
time, Tom Weber, Licensed Plumber, was called in by New Hope to snake
the lateral lines from the house toward the sewer main.
At 10:00 a.m., Wednesday, June 1 ]lh, 1998, the Contractor could not get
his cleaning equipment into the sewer line at the downstream manhole
because of the blockage in the pipe which may have been the liner itself.
The plumber that was on site had a smaller TV camera that could sit up
into the sewer main which we did install to see what kind of obstruction
was in the pipe. It was found that the liner was twisted and not fully
compressed against the sides of the existing pipe.
At that time, the Contractor had asked this office if we knew of a local
excavator who could come in a possible dig this section of pipe line up.
We suggested they contact Ben Ciccone, Inc. because we were confident
that they could perform the job, having the necessary equipment, and
personnel to allow us to excavate the pipe adjacent to the County Road.
Representatives of Ben Ciccone were onsite around 12:00 noon to start a
list of the equipment necessary to perform this job.
We then contacted Peter Nedwell of the Dutchess County Department of
Public Works, informing him of the emergency situation that was at hand
and would proceed as if the DCDPW Permit was acquired and use the
proper signage, flag personnel and whatever other means were necessary
to maintain traffic. Mr. Nedwell was in agreement with this and a
representative would be sent to the job in question.
Underground Utilities was then called for a markoff. Central Hudson
arrived at the site and marked off the gas main, which ran down the east
side of All Angels Hill Road. Camo Pollution Control was onsite and
proceeded to mark out the water main, which was east of the existing
sewer main in the Town easement.

Camo Pollution Control brought in their camera truck and equipment and
proceeded to TV the sewer line from upstream down towards the
obstruction that was found by the Contractor. The camera proceeded to a
point where it could not go any further due to this obstruction, and this
end was marked.
Town Attorney Ira Pergament was asked to come to the site to draft a
memo of understanding. This was done and signed by the Project
Manager for New Hope. Copies are included herein and include the
following:
1. New Hope would complete all contract work, which included the
reinstatement of all laterals to the satisfaction of this office.
2. The additional work required would be the responsibility of New
Hope Pipe Liners, Inc. This includes incurring the costs for Camo
Pollution Control and the excavating contractor, Ben Ciccone, Inc.
3. New Hope Pipe Liners would be responsible for cleanup and
restoration necessary to completion of the additional work.
4. New Hope Pipe Liners would be responsible for the cost by the
homeowners to repair interior damage caused by sewer lateral
backups.
We then forwarded this letter to Gerhardt Rodenberger, for his review and
acceptance.
After locating the two ends of the obstruction, it appeared that
approximately 50' of the existing sewer main would have to be excavated,
removed, and replaced with PVC sewer pipe.
Ben Ciccone, Inc. had the equipment onsite and ready to go at
approximately 2:00 p.m. on Wednesday, June 17th, 1998
Work then proceeded by finding the water main on both ends of the
blockage to insure that the water main was not damaged during the sewer
work. Workers from both Ben Ciccone, Inc. and Camo Pollution Control
worked on the sewer line replacement.

JUNE 18. 1998:
The work continued through the evening until approximately 6:30 a.m.,
Thursday, June 18th, 1998, when the final connection was made and the
two services had been completed. Backfilling operations then proceeded
in order to close-up the excavations and somewhat stabilize the site.
New Hope Pipe Liners television crew was on site Thursday morning to
reinstate the remaining two sewer laterals. They incurred some problems
with a piece of equipment breaking down causing them not to start the
lateral reinstatement until approximately 9:00 a.m., Thursday, June 18th,
1998.
Once again when trying to cut in the last two sewer laterals, they could
not fully open the laterals and they notified the plumber, Tom Weber, to
come back on site this day and try to cut open the laterals as he had done
on Wednesday evening.
The plumber arrived at the site at 4:00 p.m. that afternoon, and proceeded
to open up the sewer laterals at 230 and 232 All Angles Hill Road.
JUNE 19TH. 1998:
This work proceeded until 12: 15 a.m., Friday, June 19th, 1998. The
plumber indicated he could not proceed any further due to the fact he had
damaged all the bits he had on his trucks, however, they did not break
through the lateral to the main.
At that time, I informed New Hope Pipe Liners' foreman to advise the
homeowners that they would be out of service and try not to use the
facilities as much as possible. The Homeowners were not happy with this
situation.
During the evening more backup of sewer occurred at Mr. Belarge's house,
and he informed us of that Friday morning, at which time we were at the
site to assess the additional backup in his house.

Ben Ciccone was back on site on Friday to landscape and clean-up the
area, however, they were utilized to dig up the laterals for 230 and 232 All
Angels Hill Road so they could be cut out and replaced. The lateral at 232
was repaired at 12:30 p.m. The lateral at 230 was repaired and
completed at 5:00 p.m. At this time all sewer laterals were back on line.
Ben Ciccone proceeded to backfill these two excavations, start roadway
cleanup and stabilized the shoulder and lawn areas that were disturbed
during the repair work.
Today, Ben Ciccone was back on site to perform blacktop and landscape
restoration in the areas required.
As noted previously, New Hope Pipe Liners accepted the conditions upon
which their responsible for the cost of the additional work to replace that
section of main which had not been properly relined.
The additional costs for this office, Camo Pollution Control, and Ben
Ciccone, Inc. are as follows:

As you can see, the total for the additional work is in access of the
$37,544.00 which was the contract amount to be paid to New Hope Pipe
Liners for this project.
Please be advised that additional work may have to be done to the section
of sewer line between the downstream manhole and the section previously
replaced. On Friday June 19th, we found that the liner in this area had
separated from the walls of the existing pipe.
