1. Repair the edge of modified counters in the Tax Receiver's Office (106)
per Change Order #8.
2. Repair vinyl wall fabric seams in Lobby (100).
3. Flash both gutters at the south entries so that the gutter endcap/wall
joint is watertight.
4. Replace cracked glass pane in Lobby skylight.
5. Provide and install door silencers per Specification Section 08710,
Finish Hardware, paragraph 2.l.C.
6. Adjust strike on door l30C so that door latches smoothly and securely.
7. Repair/replace weatherstripping on door 102A.
8. Patch and point all voids adjacent to lavatory supply and drain pipes
in all Toilet Rooms.
9. Complete drainage swale at S.E. corner of site.
10. Seed ~ny bald patches in l~ areas.

Punch List
Gentlemen:
An inspection of completed work on this project was
conducted today. Present were Alan Barone, representing
Eugene Ossie, Inc., and John V. Kane and Chuck Liscum,
representing Hayward and Pakan Associates.
This punch list is generally limited to the building.
Site work will be inspected upon its completion.
A list of remaining items, noted during the inspection,
which require correction or completion, is as follows:
1. Patch the west wall electrical receptacle in the
Assessor's Office (109).
2. Repair the edge of modified counters in the Tax
Receiver's Office (106) per Change Order #8.
3. Repair vinyl wall fabric seams in Lobby (100).
4. Flash both gutters at the south entries so that the
gutter endcap/wall joint is watertight.
5. Patch the hole in the floor of Utility Room (121).
Hole contains well power conduit.
6. Grind floor patches smooth in utility Room (121)
and Mechanical Room (201).
7. Replace cracked glass pane in Lobby skylight.
8. Provide and install door silencers per Specification

Final Punch List - General Construction Contract
Gentlemen:
A final inspection of completed work on this project was conducted
by this office on May 13, 1986. Present was Alan Barone, Jr.,
representing Eugene Ossie, Inc.
During the inspection, all items contained in the letter to the Town
Supervisor from paggi and Martin, dated May 7, 1986, were reviewed
with Mr. Barone. These items are hereby included as a part of the
final punch list and a copy of this letter is attached.
A listing of items requiring correction/completion, in order to
comply with the Contract plans and specifications is as follows:
1. Repair the damaged slats in the rolling counter door assembly
at the Town Clerk's Office (104).
2. Adjust the sliding door in the Sr. citizens Room (112) so that
it operates smoothly.
3. Adjust the overhead cabinet doors of the unit kitchen in the
Sr. citizens Room (112).
4. Replace the vinyl floor tile around the drain in the Sr.
citizens Toilet (113).
s. Patch the west wall receptacle in the Assessor's Office (109).
6. Repair scratched and dented Door #114A.
7. Adjust window locking mechanism in Building Inspector's Office
(116) so that it operates smoothly.
8. Adjust overhead cabinet doors in the Tax Receivers Office (106).

extinguisher cabinet opposite the drinking fountain.
Provide and mount all fire extinguishers.
15. Adjust closers on doors 102C and 127A.
16. Provide specified glass and complete trim on window to
Judicial Clerical Room (130).
17. Replace damaged ceiling tile in Bookeeping Room (133).
18. Adjust door/jamb to Comptrollers Room (136) so that it operates
smoothly.
19. Replace vinyl floor tile around floor cleanout in Zoning Room.

20. Repair vinyl wall fabric seams in Lobby (100) and remove paste
from vinyl surfaces throughout.
21. Clean floors, glass, counters, doors and other finishes as
required in the specification.
22. Clean all debris, stakes, markers, etc. from the site.
23. Reseed all bare patches of grass areas.
24. Provide operation manuals and instructions for vault door, well
equipment, unit kitchens, SDS pumps and any other appurtenances
as required by the Specifications.
25. . Transfer, in person, all keys for the building and equipment to

Dear Mrs. Paino:
At your request, this office conducted a site inspection
of the Town Hall Sitework on May 6, 1986, with Bill Rohde
and ChuckL~cum from Hayward & Pakan, and offer the follow-
ing comments:
1) All disturbed areas to be topsoiled, seeded, and mulched.
2) A grassed swale is to be constructed from the ~orxheast
side of the entry way to C.B. #2." This is being done to
minimize the runoff onto the pa'rking areas.
3) A grassed swale is to be constructed from C.B. #2 to
C.B. #3 (rear of parking area).
4) With respect to the closed drainage system:
(a) the as-built invert of C.B. #3 shows the" elevation
to be approximately 1.3 feet lower than the design
elevation. This should be taken into consideration
by Hayward & Pakan. The Contractor should either
relay the pipe, or construct a swale to the south
to attain free discharge. If the latter is chosen,
it should be determined prior to the construction
if the swale is an appropriate design.
5) A grassed swale should be constructed on the east side
of the sewage disposal system.
6) Move the swale on the west side of the sewage disposal
system approximately 25 feet west.
7) The clay barrier on the south and.:west side of the sewage
disposal system should be reestablished.
8) The entire sewage disposal system. should be topsoiled,
seeded, and mulched.
9) A paved swale is 'to be constructed from a point IS' north
of the flagpole to the southwest corner of the ,sewage
disposal system. The swale is to be constructed at the
