Dear Mrs. Paino:
Attached are the original copies of the Contractor's
Affidavit of Payment of Debts and Claims, Affidavit of
Release of Liens and Consent of Surety to Final Payment,
submitted by Eugene Ossie, Inc.
Based on our observations, the general construction work has
been completed in accordance with the terms and and
conditions of the Contract Documents, and all items on the
Final Punch List, dated August 4, 1986, have been
completed, except the following:
1. Repair the edge of modified counters in the Tax
Reciever's Office (106) per change Order #8.
2. Repair weatherstripping on door 102A.
Also, mentioned at our meeting last week, were the
installation of the window screen and the inoperative oven
in the Employee's unit kitchen.
I have discussed these items with Mr. Barone and he
indicated that they will be repaired/installed immediately,
except for the repair of the modified counter. He has
agreed to resolving the issue of the counter by forfeiture
of payment for this item. Per Change Order #8, this amount
is $440.00. A new Change Order #10 is being processed and
will be forwarded after it is signed by Mr. Barone.

modified to reflect the reduction in payment for new Change Order
#10. Upon receipt and execution of this Change Order by you, and
completion of the above noted repair items, payment of the amount
certified can be made.
