Change Order #2
Dear Mr. Colsey and Town Board Members:
Fax: (845) 473-1962
Fax: (518) 828-3963
Enclosed please find a copy of the AIA Document G701 entitled Change Order. The purpose of
this change order is as a result of additional length of concrete installed and the installation of asphalt in
the areas of repairs made by CAMO Pollution Control. The area of additional curb was a result of
discussions during the pre -construction meeting whereby it was determined that the area in front of 30
Regency Drive required an additional 88 feet of curbing. The additional curb resulted in a cost of
$4,224.00 (88LF @ $48.00/LF = $4,224.00). The second portion of this Change Order was for the
installation of asphalt in the areas of water service replacement performed by CAMO Pollution Control
and was a line item in the original bid. These areas contained a total of 983 square feet, for an additional
cost of $6,389.50 (983 SF @ $6.50/SF = $6,389.50). Therefore, the total cost for this Change Order
amounts to 10,613.50.
Please place this on the agenda for the November 9, 2009 Town Board Meeting for approval of
Change Order #2.
Please call me at (914) 475-5749 (cell) or (845) 454-3411 Ext. 30 (office) to discuss this matter
prior to the meeting.
Very truly yours,
