Resolution Authorizing Application with NYS Office of Children and Family Services
(OCFS) for 2010
At a regular meeting of the Town Board of the Town of Wappinger, Dutchess County,
New York, held at Town Hall, 20 Middlebush Road, Wappingers Falls, New York, on January
25, 2010.
The meeting was called to order by Christopher Colsey, Supervisor, and upon roll being
called, the following were present:

The following Resolution was introduced by Councilman Beale and seconded by
Councilman Bettina.
WHEREAS, the Town of Wappinger Recreation Committee has prepared applications to
the New York State Office of Children and Family Services (OCFS) for approval of the projects
described in the Youth Development Delinquency Prevention Application, on file with OCFS
and the Dutchess County Youth Bureau
NOW, THEREFORE, BE IT RESOLVED, as follows:
1 _ The recitations above set forth are incorporated in this Resolution as if fully set
forth and adopted herein.
2. The Town Board hereby authorizes the Town of Wappinger Recreation
Committee to make application to the New York State Office of Children and
Family Services (OCFS) for calendar year 2010 for approval of the projects
described in the Youth Development Delinquency Prevention Application
prepared by the Town of Wappinger Recreation Committee which application is
on file with OCFS and the Dutchess County Youth Bureau.
3. Supervisor Christopher J. Colsey is hereby authorized to sign the application by
and on behalf of the Town and to take whatever steps are necessary to comply
with the rules and regulations of the NYSOCFS for approval of the above
referenced application.
4. The Town Board also hereby authorizes the Town of Wappinger Recreation
Committee to submit an additional application to the New York State Office of
Children and Family Services (OCFS) for approval of projects to be considered in
2010 which application is to be submitted no later than August 3, 2010.
5. Supervisor Christopher J. Colsey is also authorized to sign the 2010 application
by and on behalf of the Town and to take whatever steps are necessary to comply
with the rules and regulations of the NYSOCFS for approval of the 2010

Supervisor Christopher J. Colsey is also authorized to sign the certification
required by OCFS, in the form attached hereto.
The Town Board acknowledges that the Town will:
a. Abide by OCFS as to the propriety of all expenditures upon which the
State aide is requested
b. The project will be operated in compliance with the laws, rules,
regulations, resolutions and codes of the State of New York and its
pertinent political subdivisions
C. No youth will be denied the services or facilities of the projects because of
race, creed, color, or sex; and
d. No fee will be charged for the programs supported by State funds.

The NYS Office of Children and Family Services (OCFS) require all funded
programs to complete the Annual Report and Narrative Report for Year 2009. Enclosed
you will find the Annual Report Form(s) for the program(s) that OCFS/Youth Bureau
funded for 2009. Please complete both the Annual and Narrative Reports and be sure to
follow the instructions. The Annual and Narrative Reports are due to the Youth
Bureau on Monday, January 11, 2010 by 3:00 PM. Please return the original and
two (2) copies of the entire report by the deadline. Any report that is missing
information or not correctly completed will not be accepted and will be returned to
you so that it may be revised.
Please read the following instructions, as they will help you in completing the
reports. Remember that this is an Annual Report and must include all that you achieved
in the program year Jan. 1 — Dec. 31, 2009.
INSTRUCTIONS:

1. The Program Annual Report is a pre-printed computer generated form with your
program and agency information. If there are any changes to this pre-printed
information, please cross it out and write the new information in RED INK.
2. Record the unduplicated number of youth served for the program year Jan. 1 —
Dec. 31, 2009. Use whole numbers, not percentages. Make sure your figures in
each category (gender, ethnicity, age) add up to the total number served.
3. The monitoring section must be filled out. This is where you explain how you
self monitored your program, not how the Youth Bureau monitored the program.

Be sure to include all the information they ask for in the instructions (methods used to

4. Two original signatures are required at the bottom of the form. The person who
completes the report signs where it says "Prepared By." The Town
Supervisor/Mayor signs where it says "Certified By."
Narrative Report (instructions on reverse side of Annual Report Form)
All programs MUST submit a Narrative Report answering questions 1 - 5. The Youth
Bureau requires a narrative report be completed regardless of funding levels.
1. In item 41, you are asked to describe your major accomplishments towards your
objectives. What you need to do is list your outcomes as they are listed in your
2009 grant. Then you must clearly state the number of youth served in that
outcome AND the number and percent of youth who achieved that outcome.

Dept Only: To answer #1, please use the Outcome Information and Future Reporting
Form (Attachment C in your 2009 grant) and complete the last column indicating the
number and percent of youth that achieved each of the outcomes for the entire year.
This form will answer item #1 in the narrative report.
The narrative report must cover programming that occurred between Jan. 1 — Dec. 31,
2009.
2. In item #2, you need to state what obstacles/problems you had in meeting your
outcomes and clearly explain what steps you took to overcome them.
3. Describe your methods to evaluate the success of your outcomes (as stated in your
2009 grant). Be very clear when stating your evaluation methods. If they differ
from your grant, explain why you changed them.
4. Any areas of technical assistance that would benefit the program are stated in #4.
5. List the efforts make during the program period to secure ongoing or additional
sources of funding for this program.

PROGRAM ANNUAL REPORT
INSTRUCTIONS
■ Every program that receives funds from the Office of Children and Family Services must submit a
Program Annual Report. For Youth Bureau annual reports, Youth Bureaus must review and make
necessary changes only to the address, contact person, and telephone number. Youth Bureaus are
not required to provide any additional information on this form.
■ Programs funded through Youth Bureau must submit their report to the Youth Bureau. Municipal
Youth Bureaus submit reports to the County Youth Bureau .
■ A separate report must be completed for each application approved by OCFS. For example, if a
program received both YDDP and SDPP funds, the program must complete and submit two
reports, one for each application.
Carefully review the computer generated information on the front of this form. If any preprinted
■ information is incorrect, cross it out and provide the correct information in colored ink.
■ Statistical Report- Complete for youth under age 21. All information should be based on an
unduplicated count of youth served during the period covered by this report. Use whole numbers;
do not use percentages.

NARRATIVE REPORT:
All programs must prepare and attach to this form a narrative report which responds to the following
questions. YDDP programs that receive less than $10,000 in state aid are only required to complete the
narrative section of the Program Annual Report every three years, unless there is a significant change in
the program or when required by the Youth Bureau or the Office of Children and Family Services.

of success in achieving that outcome.
2) Describe any obstacles or problems encountered during the program period that hindered your progress
in achieving participant outcomes.
