Attached fInd a copy of a safety inspection of the Emergency Services Building as performed by
Susan Dao.
I t appears many of the items are within the ability of the Parks & Maintenance staff to address.
Please review this list with these employees and advise them accordingly.
Upon completion of the repairs, please provide a report to Susan Dao so that she may inspect such

The following items have been inspected for compliance to the N.Y.S. Building & Life
Safety Code ofN.Y.S. as part of your requested quarterly inspection of this structure. This report
list potential or current hazards that may affect the safety of any person on the site and proposed
recommendations to rectify violations for the structure.
Exterior Inspection Items:
. All exterior doors are operating sufficiently and comply with current code requirements.
. All outside lighting is currently working.
. ADA (Handicap) access parking places must be properly designated with appropriate
blue stripping and signage per those requirements. (Presently yellow painted strips)
. Several areas of curbing to parking and driveway are in need of repair, tripping hazard
posed to occupants.
. North & East side portico soffit and fascia is in need of repair (loose and can fall).
. Front East drain grate is clogged with leaves, possible drainage problem.
. Several condensing units are in need of pipe insulating repairs. Deterioration of many of
the supply and return lines has occurred over time reducing the efficiency of the units.
. South exterior door, glass pane is cracked.
. West exterior boiler room door need to be labeled.
. West exterior picnic table is rotten & unsafe to use.
Interior Inspection of Trans Care Side of Building Items:
Access giving by Frank Luvwing - Communication Supervisor
. Fire Alarm system operative through out entire building. (Mr. Luvwing states it goes off
for no reason.)
. Fire Extinguisher missing in Trans Care garage.
. Storage of several expired fire extinguishers in garage storage closet need to be removed.
. Garage slop sink has a hose connecting the palatable faucet hanging in drain waste. A
second hose is connected to a palatable faucet and ran through a ceiling tile across a
room and back down through ceiling tile in another room.
. Slop sink room and soda machine room floor tiles loose. (trip hazard)(Doors being held
open with fire extinguisher & tire.)
. Electrical room needs to clear all storage items. Exposed wires from defective fire alarm
system.
. Both boiler rooms will need to have storage items removed.
. Both boiler rooms need to be labeled.
. Boilers appear to be located on a combustible floor.
. Bathroom at entrance, vent cover missing.
. All interior doors should be labeled.
. Duct work through-out building is in need of cleaning as well as appropriate duct filters
installed.
. Receptacle cover loose in soda machine room and employee lounge.
. Exposed electrical boxes in hallways, training room, and Employee lounge.
. Garage emergency lighting appears to be inoperative.
. Main hallway has water damaged and deteriorated drop ceiling grid including missing
ceiling tiles.
. Hallway ceiling tiles have water damaged through-out.
. Main hallway near vestibule, light inoperative.
. Egress hallway need to remove obstructions.
. Garage fire wall has been compromised by the trucks backing up and damaging the wall.

. Training room missing ceiling tiles.
. Hall ceiling tiles water damaged through hall.
. Water fountain defective.
Interior Inspection of State Trooper Side of Building Items:
Access giving by Sue Benfield - State Trooper Sergeant
. Fire Alarm system operative through out entire building. (Ms. Benfield states it goes off
for no reason.)
. Duct work through-out building is in need of cleaning as well as appropriate duct filters
installed.
. West exit vestibule missing light bulb.
. Gun locker/ammunition door should be labeled. (Trooper stated label not allowed on this
door. )
This completes the Quarterly safety inspection for this structure. Please contact me with any
