The town clerk's office is frequently considered the c~nter oftown .
government. It is not only the office which is responsible fo~ reco.rdmg
activities of the town government, but it is the central place m which are
filed the documents and papers vital to the lives ofthe townspeople.
For the past twelve years, I have held the P?sition of~eputy T~,,:n. ~lerk.
When I was hired into this position I was given certain responslbll1tieS under
the former town clerk.
However, over the past two years, my duties as deputy town clerk have
increased tremendously. I now have the complete responsibility for portions
of the job that I was never responsible for in the past.
In addition to my other duties, I am now responsible for issuing all
paperwork from beginning to end for the issuance of Burial Permits, .
Certificates of Death, Certificates of Home Birth, Games of Chance
Licensing, Peddling and Soliciting Licensing, Salvage Yard Permits,
Blasting Permits, Logging Permits, Contracts and Bids, Genealogy, Oaths of
Office, Mail Machine, Copier, Public Access of Records applications, and
the new DECALS licensing system mandated by the State.
In August of this year, we were mandated by the State to sell conservation
licenses through the new DECALS system. It was a complete failure from
the beginning. I had the sole responsibility of trying to correct all the
problems associated with the computer and the two printers as well as trying
to correct the money involved. Over the weeks, many calls each day had to

SECTION I: INSTRUCTIONS

objective in completing the appraisal. Any questions should be directed to the Town Supervisor.

Be familiar with the employee's Job Description, especially the Essential Job Functions, prior to
conducting the employee's performance appraisal.

Provide the employee with an ~ Self Appraisal Form at least one week prior to the appraisal.

Prior to the date of the appraisal, review and rate the Performance Factors and then give an Overall
Appraisal Rating of the performance for the last performance appraisal period. The Evaluator should
submit the ratings to the Town Supervisor for review prior to meeting with the employee. After the
Town Supervisor reviews the ratings, the Evaluator should schedule the appraisal meeting.

Prior to the date of the appraisal meeting, complete the Goals Section. The employee should provide
input in developing the goals.

Following the appraisal meeting, the Evaluator should forward the original signed Performance
Appraisal Form to the Town Supervisor's Office to be placed in the employee's personnel file. A copy
should be given to the employee.
. The Employee Should:
. Complete the Employee Self Appraisal Form prior to the appraisal.
. The Evaluator and Employee Jointly Should:
. Review the completed Employee Self Appraisal Form at the time of the appraisal. This form is for
discussion purposes only and should not be attached to the final performance appraisal.

I enclose a check for $ 110 for the records of the Hibbert family you sent me.
I can't thank you enough for the work you did in researching my family.
What you sent me was a gold mine and connects many of the loose ends together.
All of the documents you sent mean something, and are related to each other. You
found relationships that I had no Idea existed and opened up other paths that I will be
able to follow.
My mother never told me that my grandfather had two other brothers who lived in
Wappingers and had children who were her cousins. When I was very small we
spent some time in Wappingers and I lived in Fishkill for a short time, but I never
met these people.
I did stay with my grandmothers sisters (You sent their mother Julia Bakers name on
my grandfathers death certificate). If you find any certificates on them I would also be
interested. They were Theodosia Hasbrouck married to Isaac Hasbrouck and
Pauline Howarth husband unknown. Theodosia lived in Wappingers and later in
Houghsonville. Pauline may have lived in Poughkeepsie but I'm not sure. You will
find no births but perhaps marriages and deaths They would have been alive after
1920 but married before. Theodosia had a son who was Postmaster of Wappingers
and a grandson who was Operatios Manager for IBM.
You did an outstanding job of finding these people for me. Your diligence in
researching was obviously above and beyond the call of duty and I am extremely
grateful to you and Florence. I couldn't be more happy.

The town clerk's office is frequently considered the center of town
government. It is not only the office which is responsible for recording
activities of the town government, but it is the central place in which are
filed the documents and papers vital to the lives of the townspeople.
For the past twelve years, I have held the position of Deputy Town Clerk.
When I was hired into this position I was given certain responsibilities under
the former town clerk.
However, over the past two years, my duties as deputy town clerk have
increased tremendously. I now have the complete responsibility for portions
of the job that I was never responsible for in the past.
In addition to my other duties, I am now responsible for issuing all
paperwork from beginning to end for the issuance of Burial Permits, .
Certificates of Death, Certificates of Home Birth, Games of Chance
Licensing, Peddling and Soliciting Licensing, Salvage Yard Permits,
Blasting Permits, Logging Permits, Contracts and Bids, Genealogy, Oaths of
Office, Mail Machine, Copier, Public Access of Records applications, and
the new DECALS licensing system mandated by the State.
In August of this year, we were mandated by the State to sell conservation
licenses through the new DECALS system. It was a complete failure from
the beginning. I had the sole responsibility of trying to correct all the
problems associated with the computer and the two printers as well as trying
to correct the money involved. Over the weeks, many calls each day had to
i
be made to the various departments involved - Deer Management in Albany,
the "professional" hot line in Arizona, and DEC Revenue Accounting in
Albany. We were receiving very little help from any of these departments
and dealing with them was causing frustration and very tense days and
weeks for myself as well as my two co-workers. We were spending so much
of our time trying to accommodate the hunters with a new system that didn't
work that our other responsibilities were pushed aside causing a tremendous
backlog of unfinished business and reports, etc. The responsibility to make
this program work fell totally on me since it was a mandated program and
we had to use it.
I respectfully request that you consider steps to increase my salary to
commensurate with my responsibilities and proven ability. I am now a
Grade 3, Step 9 - maxed out. I am at a dead end and have nowhere else to
go! With the amount of added responsibilities that have been added to my
position, I am requesting that I be elevated to a Grade 4, Step 9. These
additional duties present an excellent opportunity for the board to increase
my salary to the desired level without criticism. Over the past twelve years,
I have proven myself to be a loyal and responsible employee with great
ability and accuracy in my work. I have served the Town well.
Please give serious consideration to my request.

Dear Board Members,
I would like to ask you for the opportunity to speak with you regarding
the shift that will be taking place in my office when G1ady leaves on the 4th
of June, as there seems to be a slight difference of opinion regarding salaries.
I'm sure you are aware that there will be a shift in personnel, and that
the position left open by Gladys' retirement is a major one. It encompasses
not only what the day to day counter work, phones, marriage licenses, etc.,
entails, but will include doing the Town Board minutes and related correspondence,
as well as attending the Board meetings. This has always been a part of the job
description for the 1st Deputy, and why the salary differential has always been
considerable.
Connie has indicated an increase to $15,000 would be in order, I would much
prefer to place it at $17,000. Connie does, however, seem amenable to going
higher if you were to concur with me.
I would sincerely appreciate the opportunity to argue my case, and of course,
be willing to consider compromise. I also would assure you that it is understood
that the new employee would be hired at $12,500, which is your policy for new
persons. (I might add that none of this would affect my budget, in fact, there
may even be some unexpended balance at year end.)

The State Civil Service Commission has approved the
Commissioner of Personnel's Resolution 11-93, amending the
appendices to the Rules for the Classified Civil Service of Dutchess
County. (The appendices list all titles which are not in the competitive
jurisdictional class.) In the Town of Wappinger, the following title has
been approved for the exempt class:
Deputy Town Clerk (2 additional positions for a total of 3)
According to our records, the Town has only two positions in this
title. The one just approved for the exempt class is occupied by Sandra
Kosakowski. Ms. Kosakowski's permanent appointment is effective
March 6, 1995, subject to an eight to 26-week probationary period.
Our records will be adjusted to record the approved jurisdictional class
and the resulting status determination; it is not necessary for you to
submit a Report of Personnel Change confirming this action, unless the
probationary period will be waived following the minimum eight weeks.

OF THE TCMN CLERK
Office Staff presently consists of Two Full Time Deputy Town Clerks, and One
Parttime (Accounting) Clerk
PREFACE
The duties of the Town Clerk are scattered throughout approximately 14 volumes of
McKinney's laws of the State of New York, and more particularly in "Town Law",
"Agriculture & Markets", "Conservation", "Domestic Relations", and "General
Municipal Law". Deputy Town Clerks are to be familiar with the entire operation
of the Town Clerks Office, and, on a day to day basis, they must be able to handle
any area within the scope of the numerous responsibilities that the office entails.
There are presently two Deputy Town Clerks, and one Parttime (Accounting) Clerk.
(The Town Clerk may appoint no more than three deputies). These Deputies are Town
Officers. The Town Clerk is responsible for the conduct of the Office, and for all
the acts of her Deputies, as these Deputies are vested with all the powers, duties
and responsibilities of a Town Clerk (and, in fact, act in the capacity of the Town
Clerk when the Clerk is absent or incapacitated).
SCOPE OF BASIC RESONSIBILITIES
(The following must be included in the JOB DESCRIPTION of each employee in the Town
Clerks Office.)
Basic responsibilities of the Office are:
-Custody of all Town records, books and papers
-Filing Officer for the Town
-Officer charged with maintaining Registers for: Tort Claims - Notices of Hazarous

DESCRIPTION OF ACCIDENT On July 13th at approximately 3:30pm,
a customer came to the office requesting copies of budgets which are.
stored on an upper shelf in the vault of the Town Clerk's Office.
After searching through records and m~ving books around on the upper
shelf, I turned my back to leave the vault when a large 10 lb. box,
approximately 12" x 18", fell from the upper shelf, striking the upper
right side ~back near the right shoulder area. As box fell to the
ground, it also hit the lower portion of my left leg causing an immediate
black & blue spot to appear on my leg. By the next morning, I experienced
pain generating down from the right shoulder area to the upper portion of
I!!Y right arm.

the time and observed the whole incident.
Lisa Lynch, a co-worker, was standing by the Postage Meter and while watching

Registrar for the Town of Wappinger, effective with my last day of
work, Friday, October 27, 2006.
Due to more health problems and growing burdens in other areas, I
must slow my pace of work and have found it necessary to retire.
My association with the Town of Wappinger has been a pleasant one.
Beginning my employment in the Court System and eventually
moving into the Town Clerk's Office, I had the pleasure of working
with three town judges and three town clerks throughout the years.
I will miss seeing and chatting with the many people who came to
the window of the town clerk's office for service, as well as the many
close friendships formed with my co-workers.
Please accept my thanks for the opportunity you gave me to work
with all of you in meeting the challenge of the past 16 1/2 years.

EJF #1 - Mrs. Kosakowski handles a large percentage of Vital Statistic transactions.
Having 16 years of experience has made her familiar with the process and paperwork
involved. That experience is also proving to be a hindrance due to the fact that 16 years
of bad habits and misinformation have also been acquired. Many aspects regarding Vital
Statistics have been corrected and Mrs. Kosakowski is adapting. She will need to attend
classes if they are available.

EJF #2 - Mrs. Kosakowski handles all of the genealogical requests that come in to the
Town clerks office. There is very little formal organization, tracking, or set procedure in
her performing this task. Therefore, it is not likely that another employee could pick up
and carryon this task in her absence.
EFJ #3 - Mrs. Kosakowski handles half of the hunting and fishing customers that come
in. She performs this task in a satisfactory manner but has trouble with the license
printing machine.
EFJ #4 - Mrs. Kosakowski issues handicapped permits and tracks them in BAS in a
satisfactory manner.
Comments: Mrs. Kosakowski no longer processes blasting permits. Therefore, that EJF
needs to be removed from her job description.

Formal Response to Sandra Kosakowski
The following letter is a formal written response to a letter dated June 9th, 2006,
drafted and filed by Mrs. Sandra Kosakowski, Deputy Town Clerk #1.
Early in the month of June 2006, it was discovered by me that there seemed to be
a lack of accountability concerning keys that were issued out to the staff of the Town
Clerk's office. In order to rectify the situation, I asked that Mrs. Kosakowski return her
key so that I could issue her a key that was previously returned by Mrs. Florence Hannon.
Upon the return of Mrs. Kosakowski's key, all staff members would have the appropriate
keys providing the appropriate access. Accompanying the returned key, Mrs.
Kosakowski included a letter, signed and dated June 14th, 2006, a copy of which was sent
to the Town Supervisor's office, voicing disapproval of my requesting the key. A copy
of said letter is hereto attached.
On Monday, June 19th, I called the staff of the Town Clerk's office in for an
informal discussion and counseling session concerning the letter being sent to the
Supervisor's office. At the conclusion of the meeting, it was understood by both deputies
that problems within the office were to be handled within the office, and only when
satisfactory solutions could not be reached within the office were matters to be referred to
the Supervisor's office. I asked ifthere were any other problems that I needed to be
made aware of and both Deputy Clerks responded that there were not.
Approximately four hours later, while participating in the issuance of a Blasting
Permit, I came across a letter, signed by Mrs. Kosakowski and dated June 1ih, 2006, that
was placed in the Blasting file folder. The letter was placed there without my knowledge.
It contains misquotations, an abundance of exclamation points and question marks, and
false accusations. Upon the discovery of the letter, I immediately called Mrs.
Kosakowski into my office to discuss it. I asked if she believed that the events stated
within the letter actually took place. She replied that she didn't know what I meant. I
then clarified by asking if I, during the duration of the event described in the letter, ever
once yelled at her or raised my voice. She replied that I hadn't but was "very firm" with
her. At that point, two things occurred to me. First, Mrs. Kosakowski did not have a full
understanding of policy changes concerning Blasting Permits. Second, instead of
attempting to learn the new policy and workflow, Mrs. Kosakowski instead opted to draft
a very condemning letter and secretly file it for some future use.
One of the main accusatory statements claims that "no money has been received
from either the Building Department or the Fire Inspector for the payment of a blasting
permit!" Please see that I have attached the following to show the contrary:
· General Receipts from the Building Department showing that the Building
Department did in fact receive payment for the permits in question
· A photocopy of the Town Clerk's receipt, Receipt #09337 and # 09390
demonstrating the fact that the payments were also received by the Town
Clerk's office
. A memo from George Kolb, Director of Code Enforcement, stating that
fees for the Blasting Permits were in fact received by the Building
Department.
A copy of this formal response is being sent to the entire Town Board and will be
filed in Mrs. Kosakowski's personnel file. Many elements of this chain of events will
also be included in her performance evaluation for the year.

Monday, June 19t\ 2006 - The Town Clerk held a meeting for the Town Clerk's Office
staff to discuss a letter drafted by Mrs. Kosakowski. The letter accompanied a key that
the Town Clerk had requested and voiced her disapproval ofthe Town Clerk's request to
have the key returned. The focus ofthe meeting was to address the fact that the letter
was also sent to the Town Supervisor and to clear up a misunderstanding which resulted
in an overreaction. A few hours later, while searching a "blasting" file folder, the Town
Clerk discovered another letter, dated and signed by Mrs. Kosakowski, attached to recent
Blasting permit correspondence. This letter included falsehoods and exaggerations
